What is Yes I Can, and how can it help my child?
Yes I Can provides home & community-based services (HCBS) to help members achieve goals including improving confidence, enhancing social skills and building emotional resilience.
What ages do you work with?
We serve children and youth under 21 who qualify for Medicaid-based services, but eligibility depends on individual needs. Contact us to see how we can help!
Do you accept Medicaid and other insurance plans?
Yes! We accept Medicaid and most major insurances in New York. You can also check our chart here.
How do I get started with Yes I Can?
Children, youth, and their families or caregivers can connect with a Health Home care manager or call the Children and Youth Evaluation Service at 1-833-333-2937 to learn how to access Children’s HCBS.
What services do you offer?
Children: Through self-advocacy training, your child will learn to manage behaviors, navigate transitions, and thrive in the community!
Caregivers: Find 1-on-1 or group support that helps you meet personal goals, while reducing stress and promoting long-term stability.
What is HCBS, and how does it work?
HCBS (Home and Community-Based Services) are support programs provided outside of traditional clinical settings. This means services can take place at home, in the community, or even outdoors. Providers are hired from within the community so you feel ultra-comfortable!
My child is medically fragile or has special needs. Will they feel comfortable?
Absolutely. Our programs are designed for all abilities, and our staff are trained to provide personalized support. Every child participates at their own pace in a nurturing, pressure-free environment. Please note that this is NOT a site-based service.
How long does it take to start receiving services?
Once your C-YES/HHCM Representative submits IRAM referral and selects Yes I Can, It typically takes a few days to connect you to DSP and begin services.

We serve families in the following counties:
Westchester, Suffolk, Rockland, Queens, New York, Nassau, Kings, Bronx.
Is this service for caregivers or members?
This service promotes a holistic approach to involve the family as a unit.

How do I know if my child qualifies for services?
