Our personalized support is tailored to empower them in overcoming obstacles, including socialization, communication, emotional regulation, health, and wellness. Surpassing these challenges will enable your child to achieve optimal emotional and physical health within the home and community.

WHO IS ELIGIBLE?

Children enrolled in health home care management or children and youth evaluation services. (C-YES)

Anxiety, Depression, Bi-polar, Obsessive Compulsive Disorder, etc.
MEDICALLY FRAGILE (MEDF)

OUR DEDICATED STAFF

The support professional’s role is to guide and train the caregiver/child towards independence and optimal health. Collaborating with care managers, therapists, and other relevant providers, they contribute to providing individualized and appropriate care. To ensure the highest standard of care, our direct support professionals undergo rigorous screening and verification processes including state certified background checks. With educational support and a minimum of two years of experience, they are well-prepared to handle any situation that may arise. Additionally, Yes I Can offers monthly supervision to guarantee the use of proven techniques and high-quality care.
Q: What if I don’t have Medicaid?
A: Please reach out to our intake department who will help you navigate through the Medicaid eligibility and enrollment process.
Q: Can you locate a care management agency?
A: We work alongside many health home care management agencies that we can refer you to.
Q: Can I refer a specific support professional to work with my child?
A: Yes, as long as the support professional passes the qualifications requirements.
Q: Can my child receive HCBS and OPWDD?
A: If your child is enrolled in OPWDD, they cannot qualify for HCBS.
Q: Do you provide educational service through HCBS?
A: No, this service is strictly an after school behavioral health service. However, Yes I Can has a separate educational department. See more info under the Service Tab on Yes I Can website.
